PCG Technology Solutions (PCG) has been appointed to help support the Connect Well social prescribing service across Hull.
The service is being jointly managed by Citizens Advice Hull & East Riding and North Bank Forum. It aims to offer people advice and support and to provide links to community groups, services and activities which can help people to make positive changes that improve their health and wellbeing.
PCG is working with the team at Connect Well Hull to provide an online referrals and appointment booking system for 27 locations, as well as other online forms, including action plans, and a bespoke reporting dashboard.
Connect Well Hull is commissioned jointly by NHS Hull Clinical Commissioning Group (CCG) and Hull City Council and supports the Department of Health’s and NHS’ national social prescribing initiatives. Social Prescribing recognises that people’s health and wellbeing is influenced by a range of social, economic, and environmental factors. GPs, nurses and other health and social care staff often see people who can be supported by a range of non-clinical services, and a referral into Connect Well Hull will enable people to take greater control of their own health and wellbeing.
